STUDIO VISIT - If you would like an in-person studio visit, click here to download a PDF entry form which must be filled out and mailed in or dropped off in the metal box at the door of 1420 45th Street by June 7.
PAPER APPLICATION AND CD/THUMB DRIVE - If you would like to submit your entry on a CD or thumb drive, click here to download the entry form and image information sheet. Both the entry form and information sheet must be included with your CD or thumb drive and delivered to the metal box at the door of 1420 45th Street by June 7.
ONLINE - If you would like to submit your application and images completely online (no studio visit, no paper forms, no CDs/thumb drives), click here. Online entries must be submitted by June 7.
